PENGEMBANGAN MODEL PENINGKATAN KEPATUHAN
PENGELOLAAN KESEHATAN PASIEN GAGAL GINJAL TERMINAL
DI RSUD dr. H. MOH. ANWAR SUMENEP

Penatalaksanaan pasien gagal ginjal terminal meliputi terapi fisik terhadap
penyakit dasar, terapi terhadap kondisi komorbid, menghambat perburukan fungsi
ginjal dan terapi pengganti ginjal. Diperlukan kepatuhan dalam pengelolaan
kesehatan agar tidak memperburuk kondisi pasien gagal ginjal terminal. Tujuan
dari penelitian ini adalah menyusun rekomendasi model peningkatan kepatuhan
pengelolaan kesehatan pasien gagal ginjal terminal di unit hemodialisis RSUD dr.
H. Moh. Anwar Sumenep. Desain yang digunakan dalam penelitian ini adalah
explanative observational. Populasi seluruh pasien gagal ginjal terminal yang
menjalani hemodialisis di RSUD dr. H. Moh. Anwar Sumenep. Teknik sampling
yang digunakan yaitu purposive sampling dengan jumlah sampel sebanyak 32
pasien. Variabel dalam penelitian ini meliputi faktor pasien (karakteristik pasiem,
lama hemodialisis, motivasi tingkat pendidikan), faktor sistem pelayanan
kesehatan (akses pencapaian layanan kesehatan, fasilitas unit hemodialilsis) dan
faktor petugas/provider (kualitas komunikasi). Pengumpulan data menggunakan
kuesioner dan dianalisis menggunakan partial least square (PLS). Faktor pasien
mempengaruhi kepatuhan pengelolaan kesehatan. Faktor sistem pelayanan
kesehatan tidak mempengaruhi kepatuhan pengelolaan kesehatan. Faktor
petugas/provider yaitu kualitas komunikasi petugas hemodialisis mempengaruhi
kepatuhan pengelolaan kesehatan. Peningkatan kepatuhan pengelolaan kesehatan
diperlukan pada pasien gagal ginjal terminal,terutama dari faktor pasien
dibutuhkan suatu pemahaman tentang masalah fisik dan psikologis dan motivasi
pasien untuk patuh. Kualitas komunikasi yang baik antara pasien dan perawat juga
diperlukan sehingga interaksi yang terbentuk menjadi berkualitas. Peningkatan
kepatuhan pengelolaan kesehatan dengan pendekatan psikoedukasi berbasis
sistem interaksi King dapat meningkatkan kepatuhan pasien sehingga mencegah
komplikasi lebih lanjut dan meningkatkan kualitas hidup pasien gagal ginjal
terminal yang menjalani hemodialisis.

MANAGEMENT COMPLIANCE OF END STAGE RENAL DISEASES
PATIENT IN RSUD dr. H. MOH. ANWAR SUMENEP

The management of patients with end stage renal disease include physical therapy
of the underlying disease, treatment of comorbid conditions, inhibits the
deterioration of renal function, and renal replacement therapy. Required
compliance in the management of health in order to inhibit worsening the
conditions of patients with end stage renal disease. The purpose of this study was
to develop a model on improvement of health management compliance end stage
renal disease patients undergoing hemodialysis in RSUD dr. H. Moh. Anwar
Sumenep. The design used in this study was observational explanative. The
population of all end stage renal disease patients undergoing hemodialysis in
RSUD dr. H. Moh. Anwar Sumenep. Sampling technique used purposive
sampling with a sample size of 32 patients. The variables in this study included
patient factors (patient characteristics, duration of hemodialysis, the motivation
level of education), health care system factors (access to health service outcomes,
facilities of hemodialysis unit) and factors of officer/providers (quality of
communication). Collecting data used questionnaires and was analyzed using
partial least square (PLS). The statistical result showed that factors of patient and
factors officer/provider ie the communication quality affected compliance of
management health. Improved health management compliance was required in
patients with end stage renal disease, especially of patient factors required an
understanding of the physical and psychological problems and patient motivation
to comply. Quality of good communication between patients and caregivers
required to made quality interaction. Enhanced compliance with health
management approach based psychoeducation based on King interaction system
will improve patient compliance to prevent further complications and improve the
quality of life of patients with end stage renal disease undergoing hemodialysis.
