Please read the following before completing this Mutual Fund Installment Subscription Form.
1.Peserta Program Pembelian Reksa Dana Secara Berkala adalah Nasabah Mandiri Investasi yang telah
melakukan pembukaan rekening reksa dana di Mandiri Investasi.
The participants of Mutual Fund Installment Subscription Program are the Mandiri Investasi’s Customers who
had open a mutual fund account at Mandiri Investasi.
2. Jangka waktu Program Pembelian Reksa Dana Secara Berkala adalah sesuai dengan jangka waktu yang
tertera pada Formulir Pembelian Reksa Dana Secara Berkala ini.
The Investment Period of Mutual Fund Installment Subscription Program is as stated on this Mutual Fund
Installment Subscription Form.
3. Nasabah harus melengkapi Surat Kuasa Pendebetan Rekening yang telah disediakan oleh Mandiri
Investasi.
Customer must complete a Debiting Account Power of Attorney Letter provided by Mandiri Investasi.
4. Pendebetan akan dilakukan pada Tanggal Investasi per Bulan sesuai dengan tanggal yang telah dipilih
oleh Nasabah. The debiting process will be performed on the Investment Date each Month according to the date that has
been selected by the Customer.
5. Nasabah wajib memastikan dana tersedia pada rekening pendebetan pada Tanggal Investasi per Bulan
yang tertera di Formulir Pembelian Reksa Dana Secara Berkala ini.
Customer must ensure the funds are available in the debiting account on the Investment Date each Month as
stated in this Mutual Fund Installment Subscription Form.
6. Apabila terjadi kegagalan pendebetan yang menyebabkan dana tidak efektif di rekening reksa dana
pada Tanggal Investasi, maka transaksi pembelian berkala pada periode terkait akan dibatalkan dan
pendebetan selanjutnya akan dilakukan pada bulan berikutnya.
If there is a failure in the process of fund debiting that causes the ineffectiveness of the funds on the
Investment Date, the subscription of the relevant period will be cancelled and the next debiting will be done
in the following month.
7. Apabila Nasabah ingin melakukan pembelian reksa dana diluar tanggal investasi terkait dengan poin 6,
maka Nasabah wajib melakukan pembelian reksa dana sesuai dengan prosedur pembelian reksa dana
reguler, yaitu dengan mengisi Formulir Pembelian Unit Penyertaan dan mentransfer dana investasi ke
rekening Reksa Dana seperti yang tertera di Prospektus.
If the Customer wants to subscribe outside the Investment Date related with point 6, the Customer must
purchase the fund in accordance to the regular subscription procedures, namely by completing the
Subscription Form and transferring the investment funds to the product account as stated in the Prospectus.
8. Jika Nasabah ingin mengakhiri Program Pembelian Reksa Dana Secara Berkala, maka Nasabah wajib
memberitahukan Mandiri Investasi dua (2) minggu sebelum Tanggal Investasi per Bulan melalui surat
pernyataan dan Pencabutan Surat Kuasa Pendebetan.
If the Customer wants to terminate the Mutual Fund Installment Subscription Program, the Customer must
notify Mandiri Investasi two (2) weeks prior to the Investment Date each Month through a Letter of Statement
and Revocation of Debiting Power of Attorney.
9. Mandiri Investasi akan menginformasikan jika terdapat sesuatu hal atau perubahan peraturan yang
dapat mempengaruhi Program Pembelian Reksa Dana Secara Berkala ini kepada Nasabah.
Mandiri Investasi will inform the Customer should there be any changes or regulatory changes that may affect
the Mutual Fund Installment Subscription Program.
